Moorestown, NJ, and Omaha, NE, October 15, 2015–Immanuel Pathways has chosen CareKinesis to provide medication management services to participants enrolled in its PACE Nebraska and PACE Iowa programs. As a Program of All-Inclusive Care for the Elderly (PACE) organization, Immanuel Pathways provides complete medical, health and social services from a centralized location as well as in participants’ homes, enabling seniors to live independently.
“We are delighted to serve Immanuel Pathways as its partner in medication risk management,” said CareKinesis President Orsula V. Knowlton, PharmD, MBA. “We look forward to working together to reduce medication-related risk for participants while documenting improved economic, clinical and humanistic outcomes. We believe in their mission and look forward to supporting it.”
CareKinesis partners with health care organizations to provide personalized medication therapy management and pharmacy services for high-risk populations. The results are enhanced medication-related outcomes and greater operational efficiency.
Starting in October 2015, CareKinesis began providing medication management and pharmacy services to Immanuel Pathways’ existing PACE locations in Omaha, Nebraska and Southwestern, Iowa as well as the expansion site that opened this month in De Moines, Iowa.
According to Immanuel Pathways Corporate Health Care Services Manager, Jennifer Kinney, “We chose CareKinesis for their demonstrated expertise with and commitment to the PACE model of care, both its providers and recipients of care. Immanuel has an established and reputable presence for senior services in the eastern Nebraska , western Iowa and now in central Iowa. Prior to selecting CareKinesis our commitment to excellence led us to an extensive evaluation process. Both our internal stakeholders, and fellow PACE colleagues are confident in our decision to begin this exciting partnership with the professional team at CareKinesis.”
This will be CareKinesis’ first venture into the states of Nebraska and Iowa.
PACE organizations nationwide rely on CareKinesis to reduce participant hospitalizations and recidivism through the prevention of medication-related adverse events. CareKinesis accomplishes this through their proprietary electronic medication platform, EireneRx®, expert pharmacist medication management services supported by adherence packaging choices, and flexible medication access options. Medication risk mitigation is a focus of service offerings. According to Knowlton, “CareKinesis’ vision is to improve lives by optimizing the medication use process.”
